Funding & Revenue Disclosure
Medicaid Meals — a program and DBA of Vista Verde Homes 501(c)(3) nonprofit | EIN: 99-1152633 | Salt Lake County, Utah, United States Effective Date: January 2026 | Last Updated: June 2026
1. Purpose
This disclosure explains, in the interest of transparency and consistent with Federal Trade Commission (FTC) principles, how Vista Verde Homes funds the Medicaid Meals program and the limits we place on those arrangements.
2. How We Are Funded
Vista Verde Homes supports the Medicaid Meals program through charitable sources (such as grants and donations) and through a fixed fee paid by managed care plans, Social Care Networks, or other authorized entities for bona fide marketing, outreach, education, and navigation services relating to the Medicaid 1115 / Health-Related Social Needs (HRSN) meal benefit. This fee is for services rendered, is set in advance, and is consistent with fair market value. It is not based on the volume or value of referrals, leads, enrollments, or meals, or on any item or service reimbursable by a federal health care program.
3. We Do Not Accept Referral Fees or Vendor Payments
We do not charge or accept referral fees, "per-lead" or "per-patient" fees, commissions, revenue-sharing, or outcome-based payments. We do not accept advertising, sponsorship, affiliate, listing, or other payments from food vendors or any provider of items or services reimbursable by a federal health care program. Our Medicaid eligibility navigation is provided free of charge to individuals.
4. No Third-Party Advertising or Affiliate Links on This Site
This website does not display third-party advertising or affiliate links, and we do not earn advertising or affiliate commissions.
5. Our Funding Does Not Influence Our Navigation
Our funding does not influence our Medicaid eligibility navigation, screening, or referrals. We present available options neutrally, do not steer individuals to any provider, and do not recommend or prefer any Medicaid- or Medicare-covered provider in exchange for payment. Interested individuals are directed to the authorized Social Care Network/portal, where the individual chooses their own vendor.
6. Your Data Is Protected
Information you provide for Medicaid eligibility screening is used only for education, screening, and referral coordination. We do not sell it, and we do not use it to market third-party products. See our [Privacy Policy] and [State Privacy Laws notice].
7. No Endorsement
References to programs, providers, or resources are for information only and are not endorsements. Any relationship you enter into with a provider or vendor is solely between you and that organization.
8. Use of Revenue
All revenue supports the charitable mission of Vista Verde Homes.
9. Changes and Contact
We may update this disclosure; the current version will be posted with its effective date.
