This notice applies to New York State residents and controls over our general policies to the extent of any conflict for those residents.
1. Informational Purpose of This Website
This website provides general information and screening tools to help individuals determine whether they may qualify for community-based services that may be available in New York State, including nutrition support such as medically tailored meals, health-related social needs (HRSN) services, and other community-based assistance.
Completion of a screening questionnaire or submission of personal information does not guarantee eligibility, program approval, or receipt of services. Eligibility determinations are made by healthcare providers, Medicaid Managed Care Plans, Social Care Networks (SCNs), and other authorized service organizations in accordance with applicable laws and program requirements.
2. Our Role, Neutrality, and Funding in New York
Vista Verde Homes, operating the Medicaid Meals program, is an independent 501(c)(3) nonprofit information and referral resource. We are not affiliated with, endorsed by, or part of any New York State agency, Social Care Network, Medicaid Managed Care Plan, or the federal government.
Our Medicaid eligibility navigation is provided free of charge to individuals. We present available options neutrally and do not recommend, prefer, or steer individuals toward any particular provider or food vendor — you choose your own vendor through the Social Care Network or its portal. We do not charge or accept referral fees, commissions, revenue-sharing, or any payment from food vendors or other providers in exchange for referrals or enrollments. Consistent with New York Education Law § 6530(19) and related fee-splitting restrictions, we do not split fees with, or share professional revenue with, licensed providers.
Where contracted, Vista Verde Homes may perform marketing, outreach, education, or navigation services under an arrangement with a managed care plan or Social Care Network, compensated for those services on a fixed-fee basis that is not based on the volume or value of referrals.
3. New York Medicaid Health-Related Social Needs Programs
New York State has implemented initiatives to address health-related social needs through Medicaid programs authorized under federal and state law, including the New York Medicaid 1115 Demonstration Waiver. These programs may include medically tailored meals, nutrition support, housing support, transportation assistance, community care navigation, and other health-related social services, administered through Medicaid Managed Care Plans, Social Care Networks, healthcare providers, or participating community organizations operating in New York State.
4. Referral to Participating Service Providers
If you complete a screening form and appear to meet certain criteria, you may be referred to participating organizations for further eligibility review. Participating organizations may include healthcare providers, Medicaid Managed Care Plans, Social Care Networks, community-based organizations, nutrition/meal providers, and care navigators or case management organizations.
Where more than one option is available, you choose the organization or vendor that fits your needs. Referrals are intended solely to facilitate eligibility review and service coordination. Submission of information or referral to a provider does not guarantee enrollment in any program.
5. Social Care Network (SCN) Data-Sharing Authorization
New York Social Care Networks coordinate health-related social needs services for Medicaid members and other eligible individuals. By submitting information through this website, you authorize the collection and sharing of relevant information with participating Social Care Networks and associated organizations (including healthcare providers, care navigators, community service organizations, and Medicaid Managed Care Plans) for purposes including eligibility determination, care coordination, service referral, and program administration. Information will be used only for purposes related to identifying and coordinating services that may support your health and well-being.
6. Privacy and Use of Personal Information
Information you submit may include name and contact information, address/location, demographic information, responses to social-needs screening questions, and Medicaid coverage information (if provided). It may be shared with authorized organizations involved in determining eligibility and coordinating services. It will not be sold and will not be used for advertising or unrelated marketing purposes. 7. HIPAA Notice of Privacy Practices
Certain organizations that receive referrals from this website may be covered entities under HIPAA, and Protected Health Information (PHI) may be created or shared during eligibility screening and referral. Under HIPAA, individuals have rights regarding their health information, including to request access, request corrections, request restrictions on certain disclosures, request confidential communications, and receive an accounting of certain disclosures. If services are provided through a HIPAA-covered provider or program, you may receive a separate Notice of Privacy Practices from that provider. See our HIPAA Disclaimer.
8. New York Public Health Law Article 27-F Consent
New York Public Health Law Article 27-F provides special protections for certain health information, including HIV testing, HIV status, and AIDS-related information. Such information will not be disclosed without appropriate written consent except where permitted by law. By submitting information, you acknowledge that certain sensitive health information may require additional consent before disclosure, and you may be asked to provide additional authorization if a referral requires sharing information protected under Article 27-F.
9. Telehealth and Electronic Communication Consent
Eligibility screening, navigation, and referral coordination may occur through telephone, email, video conferencing, secure messaging, or online portals. By submitting information, you consent to the use of electronic communication methods to contact you and coordinate services. While reasonable safeguards are used, no system can guarantee absolute confidentiality. Participation is voluntary, and you may request alternative communication methods when available.
10. Consent to Contact
By submitting your information, you authorize Vista Verde Homes and participating organizations involved in eligibility screening and service coordination to contact you using the information you provide, by telephone, email, text message, postal mail, or secure electronic systems, for purposes including eligibility screening, referral coordination, and program information. Message and data rates may apply; message frequency varies. You may opt out at any time (for example, by replying STOP to text messages). See our SMS/TCPA Policy. Consent to receive marketing communications is not a condition of receiving assistance.
11. Medicaid Eligibility Verification
This website does not independently verify Medicaid eligibility. If you indicate you may have Medicaid coverage, verification may be conducted by authorized entities such as healthcare providers, Medicaid Managed Care Plans, Social Care Networks, or participating service providers. Individuals without Medicaid coverage may still be directed to other community programs where available.
12. Non-Discrimination Notice
Programs referenced on this website are intended to comply with federal and New York State non-discrimination laws. Services are not denied on the basis of race, color, national origin, age, disability, sex, religion, gender identity, sexual orientation, marital status, veteran status, or other protected characteristics.
